When during an IVF cycle should acupuncture be done?

There are three windows: the months before a cycle, the stimulation phase, and transfer day. The best-studied schedule is one session shortly before embryo transfer and one shortly after, both within about 24 hours of the procedure. The practice adds weekly visits through the roughly three months before the cycle and during ovarian stimulation, on the reasoning that the follicles being retrieved began maturing 90 days earlier.

Before the cycle: the preparation window

If there is time, the preparation phase is where Chinese medicine does most of its work. Weekly acupuncture for about three months, with a herbal formula and nutrition guidance, is aimed at cycle regularity, uterine blood flow, and stress regulation before any medication starts. The plan is built from the lab results your clinic already has, including AMH, day-3 FSH, and a prior cycle's response if there was one.

During stimulation and retrieval

Ovarian stimulation typically runs 8 to 14 days of injectable gonadotropins with monitoring every few days. Acupuncture once or twice a week in this phase is used for the side effects patients describe most, bloating, poor sleep, and mood, and to keep the plan on the cycle's calendar. Herbal formulas are often paused or simplified here at the physician's preference; tell both practitioners what you are taking. Some patients have a session in the day or two after oocyte retrieval for cramping and to settle the abdomen before transfer.

Transfer day: the studied protocol

The protocol most trials tested is two sessions, one within 24 hours before transfer and one within 24 hours after, using a fixed set of points on the lower legs, arms, abdomen, and ears. The pre-transfer session is aimed at relaxing the uterus and lowering anxiety; the post-transfer session at the same, in the hours when patients are told to rest. Some acupuncturists attend transfers at the clinic; whether that is available depends on the clinic and the day. A session on the day itself is not essential if it adds stress about timing; a visit the day before and the day after is the common alternative.

After a positive test

Many patients continue weekly through the first trimester, particularly for nausea, fatigue, and anxiety. Acupuncture in early pregnancy is done with the pregnancy-specific point restrictions covered in the pregnancy safety article. If the cycle does not succeed, treatment can continue into the next cycle without a break; a frozen transfer is a shorter run-up than a fresh one and the schedule adjusts accordingly.

Common questions

Is a single transfer-day session enough?

It is the most-tested schedule, and the largest trial found it did not change live-birth rates compared with sham. The practice's view is that transfer day is the least important of the three windows on its own, and the most useful as the end of a longer plan.

Should I stop herbs during an IVF cycle?

Follow your reproductive endocrinologist. Many prefer no herbs during stimulation and after transfer; the practice pauses or adjusts the formula on request and never asks you to hide it from your clinic.

Can I have acupuncture on retrieval day?

Usually not on the day itself, since retrieval is done under sedation and you will be resting. A session one or two days after is common for cramping and bloating.
